Colorado Prescription Drug Formularies change during the year, so it is always a good idea to confirm your medication before filling a prescription.
Each health plan uses a drug list, called a formulary. Use the links below to check if a medication is covered, what tier it falls in, and whether there are any rules like prior authorization or step therapy.
Kaiser Permanente
Kaiser members can review the full formulary as a PDF.
Anthem Blue Cross and Blue Shield
Anthem offers an online tool to look up medications and see tiers, limits, and alternative options.
Cigna Healthcare
Use Cigna’s pharmacy tools to see coverage, estimate costs, and review rules for specific drugs.
Elevate (Denver Health)
Elevate publishes a PDF formulary for individual and family plans.
Select Health
Select Health provides an online formulary search and a downloadable list for Colorado plans. The formulary search provides a lot of information and detail, like expected costs for each specific dosage of medication.
RMHP / UnitedHealthcare
RMHP and UnitedHealthcare use plan-specific formularies. Start with the online search tool, then confirm the correct formulary for your plan name.
Tips when checking a medication
- Use the exact drug name, including the generic name if you know it.
- Select the correct strength and form, such as tablet, capsule, or injection.
- Check the tier, since your copay or coinsurance depends on it.
- Look for notes about prior authorization, step therapy, or quantity limits.
- If a drug is not covered, ask your doctor about alternatives or a possible exception.
